Stratum for Primary Care

Same-day demand without the same-day scramble

Acute, annual, chronic, and Medicare wellness all need different slots. Stratum sorts them on the call and protects the ones you can't get back.

Same-day demand, annual wellness visits, and refill requests all arrive on one line, and the schedule fills with the wrong things.

The workflows primary care practices actually run.

01

Acute versus routine sorting

Separates today's sore throat from next quarter's physical, and protects same-day capacity from being filled by routine bookings.

02

Wellness and preventive recall

Annual wellness visits, screenings, and immunizations worked as outbound campaigns against your registry.

03

Chronic care follow-up

Books the ninety-day diabetes recheck and the post-titration visit at the interval the protocol wants, not whenever the patient remembers.

04

Refills against visit cadence

Checks last visit and remaining fills before queuing, and offers to book the overdue appointment on the same call.

Escalations that arrive complete

When something needs a clinician, the handoff carries the full transcript and clear action items — so your staff pick up mid-conversation without asking the patient to start again.

Common questions

How long does setup take?

Two weeks, with a dedicated onboarding team. Most practices start on after-hours or overflow lines and expand from there.

Do we have to change our phone system?

No. Stratum works with your existing system and phone numbers via call forwarding.

What languages are supported?

English and Spanish today, with more in development.

How is it priced?

Outcome-based. You are charged when the agent resolves the issue, which keeps our incentives pointed at resolution rather than call length.

How is our data governed?

Your data is never used to train shared models. PHI is processed for your calls alone, under a signed BAA, encrypted in transit and at rest, with US-only residency. We are HIPAA compliant and happy to walk your security team through our controls.

How do we monitor quality?

Every conversation is logged, transcribed, and reviewable. You get a flag queue and a weekly quality review during onboarding.

Is there an API?

Yes — a secure REST API, with HL7 and FHIR support for custom integrations.
